Start by going to MedXpress at the faa.gov site, create an online account,

then complete the application.

Upon successful submission of your electronic Form 8500-8, you will be given a confirmation number.

Dr. Archuleta will be able to access your exam using this number.

Bring a printout of your online application (that shows your confirmation number) with you when you arrive for your exam.

Our office will NOT be able to access your exam without the confirmation number.
